We will notify you by telephone, email or fax (your choice) if our patient/your client misses three appointments in a row from Dr. Smith's recommended treatment plan. Medical records requests are handled promptly and generally released within four working days from receipt of the written request. All medical reports (initial, reevaluation and discharge) reports are individually created. Discharge reports are generally completed within 10 working days of discharge. Dr. Smith provides independent or second medical examinations and is available to sit on chiropractic review panels. Dr. Smith currently serves on the Board of Chiropractic Examiners Peer Review Committee.
